What to bring to your first appointment?

When you come for treatment after a workplace injury please be sure to be bring the following information with you

- Name and address of employer

- Employer phone and fax number

- Name of Manager

How long will I need to come for treatments?

Treatment time will depend on the type of injury and the location of the injury. The doctor will inform WSIB which Program of Care you will be treated under. Each Program of Care designed by WSIB to care for work relate injuries comes with a minimum number of treatments and a specific timeline for treatment. This will be provided to you at your first visit.
