In Ontario, it is the law that every auto insurance policy provides Statutory Accident Benefits. The Statutory Accident Benefits Schedule is provincially mandated therefor, the coverage provided to you is standard among all insurance companies.
Regardless of who is at fault in your auto accident, Statutory Accident Benefits provide compensation to you, your passengers and/or pedestrians who suffer injury.
Our doctors are very experienced dealing with injuries sustained in motor vehicle accidents. The most common injuries we see are whiplash associated disorders, neck and low back pain, headaches, and shoulder pain.
Your insurance adjuster will need you to complete an application for accident benefits, commonly referred to as an OCF-1. The completed form is submitted by you directly to your insurance adjuster.
Call the office and inform us of your auto accident. We will ensure the doctor has ample time to review the details of your accident and your injuries. After a complete assessment and physical exam the doctor will provide you with your first treatment.
When you arrive for your initial visit after sustaining injuries in a motor vehicle accident we will need the following information from you:
- Photo ID
- Date of accident
- Name of car insurance company
- Car insurance policy number
- Car insurance claim number
- Name of insurance adjuster
- Phone and fax number of insurance adjuster
- Name of primary and secondary extended health care company (if applicable)
- Copy of primary and secondary extended health care company benefit card(s)
